For your convenience, you can print commonly used claim forms by clicking on one of the links below (which will display the form in your browser window) and selecting the print option. Other benefit-related forms are available in the BP Benefits Center at Hewitt (see link below) or directly from the claims administrator.
Except for the Employee Relocation Authorization Form, please do not complete the forms online. Instead, print the form, fill it out and mail it to the appropriate claims administrator.
Note: You can download forms as PDF files or Word documents. If your home computer doesn't have Acrobat 3 or Word under the Office 97 suite (or better), wait until you're back at work, where you'll be able to download any form you want without a hitch.
