Annual Physical (500 points) and/or Biometric Screening (250 points)
Together, annual physicals and biometric screenings provide you with important health information.
We recommend that you complete a biometric screening before having your annual physical so you can go over your screening results with your physician. Biometric screenings with Quest Diagnostics will be available April – December, 2012. See Routine Annual Physical (500 points) and/or Biometric Screening (250 points) for more information.
Note: You can earn points for either the annual physical OR the biometric screening. If you complete both, you will receive 500 points (250 for each).
To receive 500 points, your annual routine physical* must be completed with a licensed physician. If you did not complete a separate biometric screening, your annual physical should include, at a minimum, a blood test to measure your cholesterol, HDL, LDL and Triglycerides, a check of your blood pressure and updated BMI. Self-Reporting Option: You can use the Annual Physical Self Report Option within your My Points Account to immediately receive points credit. BP may audit self-reported responses.
Note for women: While your annual well-woman exam qualifies you for the 500 points, we highly encourage you to also complete an annual physical exam with your primary care physician.
Note to Group Leaders: Participating in the executive physical program will allow you to qualify for the 500 points.
* Annual routine physicals performed in-network will be covered at 100% with no copay or deductible. However, your doctor may order additional lab tests or procedures that are not considered routine. If they are not considered medically necessary, you will be responsible for the full cost, so it is important to always check with your insurance provider before agreeing to any additional testing.
