Annual Physical (500 points) and/or Biometric Screening (250 points)

Together, annual physicals and biometric screenings provide you with important health information.

We recommend that you complete a biometric screening before having your annual physical so you can go over your screening results with your physician. Biometric screenings with Quest Diagnostics will be available April – December, 2012. See Routine Annual Physical (500 points) and/or Biometric Screening (250 points) for more information.

Note: You can earn points for either the annual physical OR the biometric screening. If you complete both, you will receive 500 points (250 for each).

To receive 500 points, your annual routine physical* must be completed with a licensed physician. If you did not complete a separate biometric screening, your annual physical should include, at a minimum, a blood test to measure your cholesterol, HDL, LDL and Triglycerides, a check of your blood pressure and updated BMI. Reference Steps to becoming a healthier you to learn about the routine screenings you should get based on your age and gender.

Not all screenings are considered “routine” and may not be covered by the BP Medical Plan. See Routine Annual Physical (500 points) and/or Biometric Screening (250 points) for more information.

Self-Reporting Option: You can use the Annual Physical Self Report Option within your My Points Account to immediately receive points credit. BP may audit self-reported responses.

Note for women: While your annual well-woman exam qualifies you for the 500 points, we highly encourage you to also complete an annual physical exam with your primary care physician.

Note to Group Leaders: Participating in the executive physical program will allow you to qualify for the 500 points.

* Annual routine physicals performed in-network will be covered at 100% with no copay or deductible. However, your doctor may order additional lab tests or procedures that are not considered routine. If they are not considered medically necessary, you will be responsible for the full cost, so it is important to always check with your insurance provider before agreeing to any additional testing.

Health Advisor Call (250 points if completed by June 30, 2012)

Once you complete your Health Questionnaire, call a StayWell Health Advisor. During the call, you can learn more about the results of your questionnaire and find out which wellness activities can help you meet your individual health goals.

Call a StayWell Health Advisor to:

  • Review the results of your Health Questionnaire
  • Learn more about BP wellness activities
  • Receive answers to your health and wellness questions

The StayWell Health Advisor call is completely confidential and should last about 5 – 10 minutes. By calling a Health Advisor, you'll also have the opportunity to speak with a Health Coach for a one-time call.

Call StayWell at 1-888-343-9862. Health Advisors are available Monday through Thursday, 8:00 a.m. to 8:00 p.m. Central time, Fridays from 8:00 a.m. to 6:00 p.m. Central time and Saturdays from 8:00 a.m. to noon Central time.

Note: StayWell Health Advisors are available year-round; however, you must complete your first call by June 30, 2012 to earn 250 points.

Physical Activity Campaign (125 points each up to 250 points)

From March through November 2012, you'll be invited to participate in various physical activity campaigns designed to help you stay active throughout the year. You may register as an individual or join as a BP team.

The 2012 physical activity campaigns include:

  • Time Out for Health
    • Register: March 5 – March 18
    • Participate: March 19 – April 30
  • Route 66
    • Register: June 11 – June 24
    • Participate: June 25 – August 20
  • Run-A-Muck
    • Register: October 1 – October 14
    • Participate: October 15 – November 12

Online Healthy Living Programs (125 points each — no limit)

Complete one (or more) of several six-week online healthy living programs on various topics including:

  • Healthy aging
  • Healthy seniors
  • Healthy heart
  • Cancer fighting
  • Diabetes fighting
  • Healthy diet
  • Getting in shape
  • Healthy kids
  • Stress relief
  • Becoming smoke free
  • Losing weight

Or create a custom program to design a program that can help you reach your individual goals.

Wellness Classes (125 points each up to 500 points)

Throughout 2012, BP will offer six different wellness classes on a variety of topics including stress management, back care, nutrition, exercise and weight management. Classes will be available on-site at many BP locations as well as online. Points are earned with the completion of a quiz or survey at the end of the class.

Log onto the StayWell Online website via the homepage of LifeBenefits and click on your My Points Account to view the wellness classes scheduled to begin in April. More information on these classes will be sent separately.


Local BP Wellness Activity (125 points each up to 500 points)

Complete up to four BP-sponsored wellness activities.

For complete details on approved local activities, log onto the StayWell Online website via the homepage of LifeBenefits and check the local activity section of your My Points Account.

Financial Wellness (125 points each up to 250 points)

Your financial health is an integral part of your total wellness, so financial education should be part of a comprehensive wellness program. Effective April 1, 2012, you have the opportunity to earn wellness points by participating in web-based (and, in some locations, on-site) classes on a broad range of personal financial topics. Some topics include:

  • Retirement planning
  • Debt management
  • Estate planning
  • Planning for the cost of higher education

You and your spouse/domestic partner can also earn wellness points by completing an online confidential Financial Fitness Assessment and a Financial Coaching call to discuss the results. Offered through PricewaterhouseCoopers (PwC), the assessment will provide you a personalized report summarizing your overall financial wellness score and suggested steps you may want to consider as you work on your financial health.

You will earn 125 wellness points for each financial wellness class and both you and your spouse/domestic partner will earn 125 wellness points if either of you completes a Financial Fitness Assessment and Financial Coaching call — to a maximum of 250 points each.

Pilot Program for Weight Management (500 points after six months of participation)

Note: The FitLogix Weight Management Program has reached its capacity for enrollment.

BP and StayWell have partnered with FitLogix to offer participants of the BP Wellness Program access to a weight management and fitness support program. The Pilot Program for Weight Management will be available beginning at 7:00 A.M. Eastern time on Monday, April 2, 2012 and will be limited to the first 1,000 participants. The deadline for enrollment is the earlier of May 15, 2012 or when the 1,000-participant limit is reached.

All active BP employees, retirees and their dependents are eligible for the program, unless they fall into one or more of the following categories:

  • Pregnant
  • Type I diabetes
  • Congestive heart failure
  • Over 360 pounds

The program is specifically tailored to work with individuals who have a weight loss goal of more than 25 pounds. The Pilot Program for Weight Management will require a high level of commitment from participants for success and is designed to work with those individuals who are ready to make changes in their life. Those that participate will be eligible for 500 wellness points in the year they complete their first six months of the program.

The program will include:

  • A 52-week program based on one simple concept — balancing calories taken in by eating and drinking with the calories burned
  • A digital scale and activity meter with a wireless connection to a personal computer through an antenna that allows participants to track their fitness success
  • 52 Healthy Steps — activity and nutrition lesson modules that can be completed at the participant's own pace
  • A series of up to 12 planned coaching calls over a 12-month period to encourage, educate and support making healthy lifestyle changes
  • Access to an online dashboard and mobile application for iPhone and Android devices
  • A structured, interactive web-enabled program that offers content along with expert and professional chat opportunities

To begin the enrollment process, go to www.bpfitlogix.com. You’ll choose a password and answer a series of online intake questions. A FitLogix coach will call each participant to finish the enrollment process. This call with a coach is required in order to become fully enrolled. The deadline for enrollment is the earlier of May 15, 2012 or when the 1,000-participant limit is reached.  Remember, the program is limited to 1,000 participants.

Telephonic Lifestyle Management (250 points)

If you want one-on-one attention, this program provides a personal Health Coach to help you with:

  • Back care
  • Blood pressure
  • Cholesterol
  • Nutrition
  • Physical activity
  • Stress management
  • Tobacco cessation
  • Weight management

These professional Health Coaches will call you at a place and time convenient for you.

They will help you set goals, deal effectively with barriers, evaluate helpful resources and keep you on track for success. Earn your points when you complete at least three phone calls with a StayWell Health Coach.

You must start one of the Telephonic Lifestyle Management programs no later than September in order to have time to complete it by December 31 and earn the 250 points.

Mail-based Lifestyle Management (125 points)

If you prefer to work independently, you may benefit from this mail-based program. This six-month program provides you with a series of useful self-help educational materials, tools and activities, and additional resources that help you meet your personal health goals and become a healthier you.

You'll earn your points after you've completed the six-month program and returned a completed survey.

You must start one of the Mail-based Lifestyle Management Programs no later than May in order to have time to complete it by December 31 and earn the 125 points.

Telephonic Condition Management (250 points)

In this confidential program, participants work with a StayWell coach to get information about managing certain chronic conditions, including:

  • Asthma
  • Diabetes
  • Coronary Artery Disease (CAD)
  • Congestive Heart Failure (CHF)
  • Chronic Obstructive Pulmonary Disease (COPD)

Points may be earned by those participants eligible for the StayWell condition management benefit. While this activity may involve multiple telephone calls, points are given at the completion of the third call with a StayWell coach and are awarded in the year that the third call is completed. The three calls must be completed within a 12-month period, and points will be awarded only once within a calendar year.

Complex Case Management (250 points)

Complex Case Management is for a small percentage of the BP population who have an acute condition that requires extensive treatment, such as severe stroke, transplant, severe injury or cancer.

Participants in the Complex Case Management program receive support in navigating the health care system and optimizing their BP health care benefits. Points are given once a care plan has been created and at least one call with a Case Manager is completed. Points are awarded in the year that the call with the Case Manager is completed.

Typically, employees/retirees or spouses/domestic partners with select severe conditions are identified by the Case Manager for participation.
