Q: When and where will the Health Questionnaire be available?
A: The Health Questionnaire is available beginning February 6, 2012. You can access it via the homepage of LifeBenefits, using any computer with access to the Internet. You’ll need your Benefits Center User ID and Password. Employees/retirees should click on the employee/retiree button to be taken to StayWell Online.
Employees/retirees who want to enroll in the HealthPlus Option for the 2012/2013 plan year must complete the questionnaire by 11:59 p.m. Central time on March 2, 2012.
Q. Can my spouse/domestic partner complete the Health Questionnaire?
A. Yes. All spouses/domestic partners of BP employees and non-Medicare eligible retirees are invited to complete the Health Questionnaire. In fact, spouses/domestic partners must complete a Health Questionnaire between February 6 and March 2, 2012 in order to be eligible for the HealthPlus Option for 2012/2013.

Q: Why do I have to click through so many web pages to access the Health Questionnaire?
A: There are several steps required for accessing the Health Questionnaire. Each step is necessary to ensure that you receive "credit" for completing the Health Questionnaire to be eligible for the HealthPlus Option. And, these steps help us keep your personal information private and protected.
Q: Can I complete the Health Questionnaire by phone?
A: No. It’s available online only. However, if you do not have access to a computer at work or at home, call the StayWell HelpLine at 1-888-343-9862 to request a printed copy. The paper version must be completed and postmarked no later than March 2, 2012. The online method is preferred.
Q. Do I need specific health information to complete the Health Questionnaire?
A. Completing the Health Questionnaire does not require you to know your detailed health history. Knowing the following numbers, which you can get from a routine physical exam, will help strengthen your results, but you can still complete the Health Questionnaire even if you do not have all the information:
- Measurements: height, weight (BMI)
- Blood pressure
- Cholesterol: HDL, LDL, Triglycerides
If there's a question you cannot answer, just answer "not sure." You can return to the Health Questionnaire at any time throughout the year to update your answers and health information.
Q. If I start the Health Questionnaire but need to stop, can I save my answers and come back later to finish it?
A. Yes. Click either the "End Assessment" or "Logout" buttons located at the bottom of the left side menu. This will ensure that your answers are saved. But remember, you must complete the Health Questionnaire by March 2, 2012 in order to be eligible for the HealthPlus Option.
Q: Can I print my questionnaire results?
A: Yes. Just click the printer icon on the last page of the assessment.
Q: How can I confirm that I received “credit” for completing the Health Questionnaire and that I’m eligible to enroll in the HealthPlus Option?
A: When you enroll via the online BP Benefits Center, you will see HealthPlus as one of your enrollment options (if you are eligible). Note: Employees/retirees who have not completed the Health Questionnaire or who are otherwise ineligible will not see HealthPlus listed.
Q: I have some questions about my personal health assessment, who can I talk to about it?
A: StayWell Health Advisors can review your Health Questionnaire results and answer any questions you may have. Health Advisors are available Monday – Thursday from 8:00 a.m. – 8:00 p.m., Central time, Fridays from 8:00 a.m. – 6:00 p.m., Central time and Saturdays from 8:00 a.m. – noon, Central time. Contact StayWell directly at 1-888-343-9862.
Additionally, you may earn 250 points for 2012 if you contact a Health Advisor by June 30, 2012.
